HOSPICE - RN
JOB SUMMARY: As a member of the multidisciplinary team, the Hospice Nurse works under the general direction of the Hospice Clinical Nurse Manager, Medical Director and the attending physician in the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families.
RESPONSIBILITIES: Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician’s orders, plan of care and Hospice of Clay County policies and procedures.
D. PROFESSIONAL REQUIREMENTS:
1. At least two years of varied work experience as a professional nurse, and preferably one of the two years in an acute setting.
2. Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
3. Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
4. Adheres to dress code, appearance is neat and clean.
5. Completes annual educational requirements.
6. Maintains regulatory requirements.
7. Maintains patient confidentiality at all times.
8. Reports to work on time and as scheduled, completes work within designated time.
9. Wears identification while on duty.
10. Attends annual review and in-services.
11. Attends IDG/staff meetings as appropriate.
12. Reads and signs interdepartmental memos/notices.
13. Complies with all organizational policies regarding ethical business practices.
14. Participates in performance improvement and continuous quality improvement (CQI).
15. Represents the organization in a positive and professional manner in the community.
16. Communicates the mission, ethics and goals of the organization.
